They choose based on where the cancer started and its microscopic type, its stage and the goal of treatment, biomarkers such as HER2, EGFR or MSI, genomic risk scores in some cancers, and your organ function and fitness. The response is then checked with scans after 2 to 3 cycles, and treatment is changed if it is not working.

Few moments in cancer care feel as uncertain as starting chemotherapy. You want to know that the drugs entering your body are the right ones, and that you are not losing precious time on a treatment that will not help. So it is natural to ask how you can know which chemotherapy will work best for your cancer type.

The encouraging news is that chemotherapy is no longer chosen by guesswork. Decades of clinical trials, combined with modern pathology and biomarker testing, allow oncologists to match treatment to the cancer far more precisely. Here is how that choice is made and what you can ask.

Why There Is No Single “Best” Chemotherapy

Cancer is not one disease. A breast cancer and a stomach cancer behave differently, and even two breast cancers can respond to entirely different drugs. For each cancer type and stage, clinical trials have identified chemotherapy regimens that work better than others, and those regimens form the starting point. The chemotherapy that will work best for you is the one that fits your cancer’s biology, your stage and your body.

Factor 1: Where the Cancer Started and How It Looks Under the Microscope

The organ of origin and the histology, meaning the type of cell the cancer grew from, are the foundation of every decision about which chemotherapy will work best. Adenocarcinoma and squamous cell carcinoma of the lung respond differently to certain drugs, and small cell lung cancer is treated with completely different regimens from non-small cell lung cancer. The idea is appealing, but current evidence has not shown that choosing chemotherapy this way improves survival, and major oncology societies do not recommend these assays for routine use outside clinical trials.
